Passport Canada has created electronic forms in PDF format that, once printed, can be completed by hand or typed.

Lost, stolen, damaged, destroyed or inaccessible document If an application for a new passport or travel document is made because a passport or travel document has been lost, stolen, damaged or destroyed or is inaccessible, you must include in the application form a completed "Statutory Declaration" (PPTC 203).
Printing requirements- Adobe® Acrobat® Reader® version 5.0 or later.

Form is printed on white, 8 ½ x 11 inch (216 x 279 mm) bond-quality, non-glossy paper.

Set the PAGE SCALING at NONE and select the AUTO-ROTATE AND CENTER option, both found under PAGE HANDLING when the Print dialogue appears.
Important: Poorly printed forms are not acceptable.
