1. Registration Deadline
Registration ends Wednesday, September 27, 2006 or once capacity has been reached. Registration in person will only be accepted if space allows.
2. Simultaneous Translation Services
Simultaneous interpretation in English and French will be available throughout the CFL Annual Fall Seminar.
Receiver devices (headsets) will be available to delegates at no charge. It is necessary to leave one item of photo identification, such as a driver's license, to obtain a headset. Your identification will be returned to you when the headset is returned.
3. Registration Fee and Methods of Payment Information
REGISTRATION FEES
Fees are quoted in Canadian dollars.
One-day Seminar, October 4: $195.00 CAN
Registration fee includes one continental breakfast, one luncheon, two networking refreshment breaks, Agatha Bystram Award for Leadership in Information Management presentation *
* Please note that the 2006 Agatha Bystram Award for Leadership in Information Management will be presented in conjunction with the 2006 Information Management Community Recognition Awards, during the Information Management (IM) Symposium, on Monday, October 2 at 10:30 a.m., Ottawa Congress Centre, 55 Colonel By Drive, Ottawa ON. CFL delegates are cordially invited to attend. Please indicate on the registration form whether you will attend the Awards presentation.
Delegates may register in one of two ways:
1. Online
Complete the secure electronic Registration Form and submit it before Wednesday, September 27, 2006.
2. Mail
Use the downloadable registration form if paying by cheque/money order and mail the completed version in time to be delivered before Wednesday, September 27 to the Conference Manager.
Please note:
- Telephone registration cannot be accepted.
- All applications must be accompanied by full payment of fees and all pertinent documents and information.
- Registrations will not be confirmed until payment is received.
- A receipt for registration fees will be issued upon request.
METHODS OF PAYMENT
Methods of payment include VISA, MasterCard, American Express, Discover, money order and cheque (payable to HPB Association Management Services Inc.). Charges to your credit card will appear on your statement as "HPB Association Managemen".
Only VISA and MasterCard will be accepted for onsite registration.
Note for Government of Canada, Crown Corporation and Agency employees: Payment must be made through your Canadian Government Department. Applications must include Department Number, IS Organization Code, and IS Reference Number.
4. Replacements / Substitutions
Registration may be transferred to another individual if a written request is received from the registered delegate. Submit requests by email or fax to the Conference Manager.
5. Cancellation and Refund Policy
Only written requests for cancellation received by Wednesday, September 27 will be refunded less a $75 administration fee. Requests for cancellations received after Wednesday, September 27 will not be refunded. Registrations not paid in full as of Wednesday, September 27 will be cancelled without refund unless guaranteed by a credit card. All requests should be submitted by fax or email to the Conference Manager.
6. Special Requirements
To indicate dietary restrictions, fill out the appropriate section on the registration form. Conference facilities are fully accessible for participants with physical challenges. If you require assistance onsite, please contact the Conference Manager.
