You may use this form to inhibit the issuance of returns. In addition, you may also cancel instructions that you previously set on your account.

Copies of Notices and Statements
Use this form if you want to request a copy of Notices and/or Statements previously issued to your account.

Customized Statements
Use this form if you want to request a Statement of Arrears and/or a Statement of Interim Payments for a period different than the one covered by the automated monthly statements we issued for your account.

Additional Remittance vouchers
Use this form if you want to request additional remittance vouchers.

Interest review
Use this form to request an interest review for your account or to request a Statement of Interest for a specified period for your account (with the exception of Brewers).

Payment search
Use this form when you want to request a search for a payment that has been made but has not been credited to your account.

Refund
Use this form if you want to request a refund of an excess credit amount from your account.

Transfer of a credit
Use this form when you want to request the transfer of a credit within or between an other levies account, or from an other levies account to the corporation, payroll, and/or goods and services tax account(s), to correct a misallocated payment.
