New Horizons for Seniors – Elder Abuse Awareness Funding

Call for Proposals

This Call for Proposals will deliver contribution funding for projects that help to raise awareness about the abuse of older adults.

You must complete the Application Form and prepare a Detailed Project Proposal. The information contained in the Guide for Applicants provides you with instructions on what the Application Form should include and the required format for your Detailed Project Proposal.

You can download the Application Form in your choice of one the following formats:

MS Word format (826 KB)
This format can be saved and completed on your computer, and then printed.
PDF format (178 KB)
This format must be printed and completed by hand. To download and print PDF files, use the free Adobe Acrobat Reader.

A checklist is provided in the Application Form to assist in ensuring that all elements of the application have been included. Incomplete submissions will not be considered for funding.

NOTE: If your organization is located in the province of Quebec , you must also complete the M-30 form and attach it to your proposal. You can download the M-30 form in your choice of one the following formats:

How to Submit Your Proposal

Your complete package must be postmarked by December 14, 2007 and must include:

one (1) original, completed and signed Application Form and Detailed Proposal;
AND
one (1) electronic version (on CD or diskette) of the completed Application Form and Detailed Proposal.

E-mailed or faxed copies will not be accepted. It is preferable that the electronic version of the proposal be submitted in Rich Text Format (RTF). The original paper version must be signed by designated authorities. The signature block is included in Part E of the Application Form.
