Forms for making a disclosure

Disclosure of wrongdoing in the public sector is a courageous and commendable act of service.

Before contacting us to disclose wrongdoing, we encourage you to read the section Disclosure of wrongdoing. 

To make a disclosure of wrongdoing with the Office of the Public Sector Integrity Commissioner, you have to provide the following information, preferably in writing:

1. The nature of the wrongdoing;
2. The name of the person(s) alleged to have committed the wrongdoing, if known;
3. The date and description of the alleged wrongdoing, and any other relevant information;
4. Your name, telephone number and address so that the Commissioner’s Office may contact for more information.

We strongly encourage you to use the form below to make a disclosure, and bring it in person to the Office or send it by fax to: 613-946-2151. This will greatly help us in reviewing your disclosure.

Please take note that...

• We do not accept formal disclosures via e-mail. 
• You may only provide information that is reasonably necessary to make the disclosure.
• You must not disclose Cabinet documents or information that is subject to solicitor?client privilege. (If such information is disclosed to the Office, it cannot be used in the investigation).
• You must follow established procedures or practices for the secure handling, storage, transportation and transmission of information or documents.

Forms for filing a reprisal complaint

The Office of the Public Sector Integrity Commissioner has exclusive jurisdiction to investigate reprisal complaints related to a protected disclosure.

To file a complaint, simply complete the form below and bring it in person to the Office or send it by fax to: 613-946-2151.

Please take note that...

• We do not accept formal reprisal complaints via e-mail. We also cannot investigate anonymous reprisal complaints.
• Any complaint of reprisal must be filed no later than 60 days after the day on which the complainant knew, or in the Commissioner’s opinion, ought to have known, that the reprisal was taken. (The Commissioner has the discretionary power to extend this period depending on the circumstances.)
