- Applicants from New Brunswick, Nova Scotia and Ontario - You must use the template provided.
- Background Check Consent form - Individuals should print, fill, sign by hand and scan the completed Background Check Consent Form before uploading it on the Senate Appointments Self-Serve system. You must use the template provided. Other forms or background check results from other organizations will not be accepted.
- Curriculum Vitae*
- Reference letter* - Referees should print, sign by hand, and scan their completed reference letter prior to sending it back to the applicant.
*It is strongly recommended that you use these templates. You may use a different format as long as you include the same information and respect the limits on length.
