The TSB was created on March 29, 1990, and we’ve spent the past 25 years investigating accidents and publicly reporting on what we’ve found. Across Canada and across the world, we’re recognized as a leader in accident investigation. When something goes wrong—whether it’s on our waterways, along our pipelines or railways, or in the sky—the TSB is there to find out what happened, why it happened, and what needs to be done so that it doesn’t happen again.
